LICENSED PRACTICAL NURSE

QUALIFICATIONS

Minimum:

Must have knowledge of nursing and focused assessment skills. Must maintain licensure requirements according to the Ohio State Board of Nursing licenses. Must have knowledge of nursing of individuals with developmental disabilities. Must be certified to dispense medications (OOPNES/NAPNES). Must be certified in CPR. 

Must be able to access and use the computer, agency internal website, Internet Explorer, Microsoft Outlook and Word.

Must have a valid Ohio driver’s license and an insured personal vehicle for use while on agency business. Must have a good driving record. Must have a history of satisfactory job performance. Must have a record of reliable work attendance.

Must be able to meet all requirements and obtain all certificates required to perform the job or any aspect of the job, and to access funding for performance of any aspect of the job.

Preferred:

2 years experience working with individuals with developmental disabilities. Previous experience in a Medicaid/Licensure facility.

RESPONSIBILITIES

Provide emergency care for illnesses and injuries. Provide nursing care and implement treatment regimen to assigned individuals. Assure accuracy for all medical records in accordance with Medicaid regulations, Ohio DODD regulations and the Ohio Board of Nursing rules. Works closely with the Registered Nurses/Supervisor, Program Supervisors, Physician, and pharmacy personnel as part of a medical team. 

Evaluate consumers’ physical/health problems in coordination with supervising Registered Nurse and screen for communicable diseases. Develop and provide inservice training to staff, residents, and others as related to plan of care. 

Effectively communicate with verbal and non-verbal individuals. Implement instruction, behavior support strategies, and training techniques as directed. 

Assist with direct care of consumers, e.g., bathing and positioning of non-ambulatory consumers, toileting and feeding. Administer prescription medications and treatments as required in all program areas, e.g., school, OVCs, group homes, and ICF/IID. Participate in the delivery of services through Individual Service Plans (ISP), and nursing care plans. 

Coordinate and schedule all medical, dental, and vision appointments and follow through with recommendations. Transport to and assist with all medical appointments. Responsible for nursing stock items and ordering medication. Train individuals in self-administration of medications and human sexuality. Obtain lab specimens, notify appropriate labs, and arrange transportation. Inventory medical supplies and assist with requisitions as necessary. 

Must be available when assigned and for on-call duty in emergency situations. Provide nursing staff coverage for vacations, call-offs, etc.

HOURS:

7:00 am-3:30 pm. Variable and flexible as needed to meet the needs of individuals.

SALARY:

LEVEL 5N

PROBATION:

280 calendar days (initial); 90 calendar days (non-initial)
