COOK/AIDE PT
QUALIFICATIONS
Minimum:
Must have a high school diploma or GED. Must be able to transfer/lift adult residents. Ability to work with residents with developmental disabilities and communicate effectively with both verbal and non-verbal residents. Cooking skills, including knowledge of different food consistency. Ability to work with kitchen appliances and inventory records. Knowledge of safety and sanitation procedures. Computer literate.
Must possess or be able to obtain CPR and First Aid certification and Delegation of Nursing Tasks training. Must be able to lift up to 25 pounds.
Must have a valid Ohio driver’s license and an insured personal vehicle for use while on agency business. Must maintain a good driving record. Must have a history of satisfactory job performance. Must have a record of reliable work attendance.
Must be able to meet all requirements and obtain all certificates required to perform the job or any aspect of the job, and to access funding for performance of any aspect of the job.
Preferred:
Experience in working with persons with developmental disabilities.
Ability to use spreadsheet software (Excel, etc.)
RESPONSIBILITIES
Prepare meals, per menus, and in accordance with resident diets.
Document food usage, menu substitutions and food orders. Complete food inventory and grocery shopping.
Maintain clean and sanitary work environment. Complete all assigned cleaning responsibilities.
Provide direct care of individuals in a residential setting, including lifting, transferring, positioning, feeding, bathing, grooming, and toileting.
Implementation of Ohio Individual Service Plans (OISPs). Knowledge of Medicaid requirements pertinent to the position.
Maintain various records, electronically as required, including progress notes, frequency charts, food inventory, treatment sheets, incident reports, etc.
Transport residents to activities and medical appointments.
Participate in staff meetings and in-services.
Housekeeping including after-activity clean-up; cleaning of bathrooms, laundry, kitchen, dining room, residents’ rooms, closets, and dressers; and laundry.
HOURS:
Generally, 21.5 hours per week. Flexible as needed.
SALARY:
Level 4.5
PROBATION:
280 calendar days (initial); 90 calendar days (non-initial)
